Terms and Conditions

These Terms and Conditions (“Terms”) govern the provision of performance nutrition consultancy and professional services (“Services”) by Evie Cook trading as Replenish and Rise (“I,” “my,” “me”) to you (“Client,” “you,” “your”). By booking or participating in my Services, you agree to these Terms.
1. Services
1.1 I provide personalised performance nutrition consultancy, including:
Individual consultations (online or in-person).
Tailored nutrition plans.
Ongoing support for clients with performance goals.
Nutritional psychoeducation.
1.2 I may provide specialist guidance for clients with a history of or current disordered eating or eating disorders.
1.3 I also offer:
Nutrition supervision sessions for dietitians and nutritionists, to support professional development and reflective practice.
Policy, pathway support, and education for organisations requiring expertise in eating disorder-related service development.
Guest lecturing on topics within my expertise.
1.4 These services are provided by me, Evie Cook, a registered, qualified dietitian. They do not replace medical diagnosis, medical treatment, or psychological care.
2. Age Requirements and Parental Consent
2.1 I provide Services to individuals aged 11 years and over.
2.2 For Clients aged 11 to 17 years, Services will only be provided with written informed consent from a parent or legal guardian prior to commencement.
2.3 For Clients under the age of 18:
· A parent or legal guardian must complete and sign all required consent documentation.
· A parent or legal guardian may be required to attend the initial consultation and/or review sessions where appropriate.
· I reserve the right to determine the level of parental involvement required, in accordance with professional, ethical, and safeguarding standards.
2.4 I will assess a young person’s capacity to engage with nutrition consultancy and provide informed assent. If I believe a young person is unable to engage safely or appropriately, I reserve the right to decline or discontinue Services.
2.5 Where safeguarding concerns arise, I will act in accordance with UK safeguarding legislation and professional duty of care obligations, which may include escalation to appropriate services without prior consent where necessary.
3. Client Responsibilities
3.1 You must provide accurate and complete information regarding your health, medical history, lifestyle, training, and relevant background.
3.2 You acknowledge that:
· My advice is educational and supportive in nature.
· Nutritional guidance requires commitment to sustainable behavioural change for optimal results.
· If we discuss use of supplements during your consultation, you have a responsibility to liaise with your GP for their suitability and safety for your personal needs.
· You remain responsible for consulting your GP or relevant healthcare provider regarding medical concerns.
3.3 If you have, or have had, an eating disorder, disordered eating patterns, or Relative Energy Deficiency in Sport (RED-S), you must inform me prior to assessment so I can determine whether my Services are appropriate or whether referral to specialist care is required.
4. Disordered Eating, Eating Disorders, and RED-S Support
4.1 As a registered dietitian, I take a compassionate, evidence-based approach to supporting clients experiencing disordered eating patterns.
4.2 I do not accept referrals for individuals whose needs are likely to require multi-professional specialist eating disorder treatment. Suitability will be determined on a case-by-case basis.
4.3 In exceptional circumstances, I may accept referrals where appropriate multi-professional collaboration is in place (e.g., GP, psychiatrist, psychologist, or specialist eating disorder service).
4.4 If, during our work together, I believe your needs exceed the scope of my practice, I may recommend referral to appropriate healthcare services. Where possible, I will seek your consent prior to referral. However, where there is a serious risk of harm to yourself or others, I am professionally and legally obliged to escalate concerns without consent in line with my duty of care.
4.5 My Services are not a substitute for medical, psychiatric, or psychological treatment. I reserve the right to pause or discontinue Services if I consider specialist support to be in your best interests.
5. Payment and Cancellation
5.1 All consultations and services must be booked in advance through our website or agreed upon platforms.
5.2 Payment for services is required as follows:
1:1 nutrition consultations and supervision sessions may be paid for as single sessions or as part of a package (see 4.4 below).
Policy and pathway support and guest lecturing require full payment upfront, with any agreed expenses (e.g., travel, accommodation) covered by the Client.
5.3 Cancellation Policy:
· Cancellations require at least 48 hours’ notice. Late cancellations or missed appointments will require you to pay the full fee for the missed session.
· This cancellation policy applies equally to supervision sessions booked by dietitians and nutritionists.
· For clients on package deals, any session cancelled with less than 48 hours’ notice or missed without notice will still be deducted from your package allowance and cannot be rescheduled.
5.4 Package Payments and Early Termination:
Packages may be paid for:
Upfront in full, or
In instalments, as agreed in writing at the time of booking.
If you choose to end your package before completing all sessions, any sessions already attended will be charged at the standard single-session rates (£120 for the initial assessment, £100 per follow-up).
Any discounts applied for package pricing will no longer apply, and the cost of used sessions will be recalculated at the standard rates.
An admin fee of £50 will be applied to cover processing and administrative time.
If the recalculated amount for sessions used plus the admin fee is less than the total amount paid, the remaining balance will be refunded.
Example: If you purchase the “Transform” package (1 initial assessment + 7 review sessions totalling £680) but decide to leave after attending the initial assessment and 3 follow-up sessions:
Standard rate calculation: £120 (initial) + £100 x 3 (follow-ups) = £420
Add admin fee: £420 + £50 = £470
Refund: £680 – £470 = £210
5.5 Instalment Plans: If you are paying in instalments and choose to terminate your package early, you are still liable for the recalculated cost of sessions already attended at standard rates plus the admin fee. If the recalculated cost exceeds the amount you have already paid, you must pay the outstanding balance before termination is confirmed.
5.6 Package sessions must be used within 6 months of purchase unless agreed otherwise in writing.
5.7 I reserve the right to cancel or reschedule appointments if necessary.
6. Liability
6.1 I am not liable for:
Any injury, illness, or condition arising from the implementation of advice without appropriate medical supervision.
Failure to achieve expected results, as outcomes depend on individual circumstances and adherence.
7. Confidentiality
7.1 All information you share with me is treated confidentially, except where disclosure is required by law (e.g., if there is a risk of harm to yourself or others).
7.2 Your personal information is handled in accordance with my privacy policy.
8. Intellectual Property
8.1 All content on this website (including text, graphics, resources), as well as resources given to clients during nutritional sessions, is owned by Evie Cook under Replenish and Rise and may not be copied or distributed without permission.
9. Governing Law
9.1 These Terms are governed by the laws of the United Kingdom and any disputes will be subject to its jurisdiction.
